How It Works:

Prior to discharge, a patient schedules a post-discharge follow-up call with a Carenet nurse. (If a patient does NOT schedule within 24-72 hours, a Carenet post-discharge representative calls to set it up.)

24-72 hours after discharge, the health system provides Carenet with the patient’s discharge file.

A Carenet nurse calls the patient and asks a series of follow-up questions, documenting their responses.

After their post-discharge call, the patient is sent daily texts up to 30 days after discharge with questions about their health status.
